CLASS REGISTRATION FORM

TO REGISTER:

1. Drop off or mail your complete Registration Form with deposit to our office at

Limestone Cr.126, Unite 2, Toronto, ON M3J 2S4.

2. Registration Forms are processed on a first come first served basis.

3. Registration forms must be accompanied by $20 deposit payment. Method of Payment:

Cash, Cheque

REFUND & CANCELLATION POLICY:

1. If a course is cancelled due to low enrolment, you will be notified one week prior

to the start of the course & a full refund will be issued immediately.

2. Registrants may cancel, without penalty, at the day of first class.

A deposit will be charged should you cancel after the first class.
